Custom Review/Comment form in custom post type

  • Hi all,
    My scenario is:
    I have created a custom post type like product. I want to add a review from as like comment from of post . But I need some more fields like rating and some other fields.

    Summery :
    # Custom Post Type
    # Custom Review or comment of that custom post type.

    Please advice me. How can I do this?
