Custom Reset Password Form

Do you refer to login form or password reset form?

Because the reset form request only email, and for login form users can login with either username or email.

I refer to the password reset form [ultimatemember_password]. I would like to customize it. Placeholder in input and texte above it. I would also need to customize other pages like the account page for example [ultimatemember_account]. How would I be able to do so?
