Custom Options are gone

I have been utilizing the basic plug in for some time, and all of a sudden the custom options for my products no longer show up on my packing slip or invoices.
For example, I have a monogram store that requires my customers to input their initials in. I would like this information to be back on the packing slip. It was until recently.
Is there a reason why? Is there a way to fix this?
