Custom Map in Forms

Hi.

I’m making a new form where i have my customers select a product, type their names and i need them to pick an already defined marker in a map. I already use WP Google Maps in my website.
I need to show this same map in the form and in submissions to show me which marker people choose.

Is this possible with the personal paid version?
