Custom Login plugin is very good.

Really delighted with this plugin: Custom Login. Really big thanks to the developer ??

One word of warning though, make sure you tick the ‘activate’ ‘toggle’ box at the beginning of the plugin userface – I didn’t realise we had to tick that and spent an hour tearing my hair out as to why it was having no effect. Once I ticked that little box all was well and beautiful.
