Custom image upload field on custom post type

Need someones help as this is driving me loopy.

Im working on an image upload field based on code found here:

I working on a custom post type ‘communities’; currently the metabox shows up in the admin area and images are uploaded to the server but the meta info is not saved for some reason and hence i cant retrieve the image to display in admin or on the front end.

Any suggestion is welcome !
