Custom Form Field Placeholder Appearing at top of Page

  • I added Placeholders to the form fields:
    Easy Appointments>Settings>Customize>Custom Form Fields
    I expanded Email and typed Email as the Placeholder.
    Did the same for the other 3 fields. Applied each and then Saved.
    When I look at page on front end, it looks like the placeholder for the Description field is way up at the top of the page, nowhere near the form. Also it looks like the Phone field label is up there too.
