Custom CSS popup image

Hello,
I have a premium gold plan and am trying to add an image into my pop-up form. In the custom css box on the Appearance Tab I’ve added:
<img src=”https://www.mywebsitecom/wp-content/uploads/2015/12/my-image.jpg” alt align=”none”/>
But it’s not showing up. Do I need a different code here?
