Custom “Create Event” PHP form call results in a dead “locations” drop-down

I embedded the “events-manager/templates/forms/event-editor.php” into the user profiles via the “include” php command, but unfortunately the resulting form has an issue. The drop-down box for the user to select the location of the event isn’t populating with the pre-defined locations, and is instead remaining blank. As it is, users can’t submit this form because a location is mandatory (my choice).

Do you have any advice about how to re-enable the locations portion of the form?

(Side note: I haven’t modified any of the original plugin files).
