Custom contact form – no e-mail being received

I started out using the GUI/shortcode method of creating a form, and found that I needed to configure SNMP mail to get a form submission to result in a e-mail being received in my account. Must be an issue with PHP send mail with my host. In any case, SNMP worked so all was good! Next, I decided to create a custom form, which worked well, apart from there never being an e-mail received. A submission finds its way into WordPress just fine, just no e-mail. Are there additional hidden form tags required to enable SNMP in a custom form?

There is also an issue if you configure an image upload as a required field – even if an image is selected, it still reports that it is missing.
Any help would be appreciated. I messaged the author through the support form in the app, but no reply so far!
