Custom Code (Pore Clogger Checker)

I am looking to have a page added to my wordpress that includes the shown following example

The last person that added it did it horribly to where it does not match with my themes layout – it looks quite bad as you can see.

In addition, I created a staging site as my theme requires an update, I updated on the staging site and the input box vanishes

The person who installed the code must have edited the parent theme files vs. child and clearly did not care what it looked like either.

Does anyone know of where or who I can hire to install this to not only be operational but to look more similar to the first link provided?
