Custom aria-labels (for accessibility)?

I use this plugin to link to external pages that open in a new tab. Per accessibility guidelines, all links that open in a new tab / window need to have a label that screen readers would announce.

Can this plugin be modified to allow for custom aria-label tags on a per-link basis? Here’s a mockup of what it could look like: https://i.imgur.com/M1hTa6B.png.
