CSV won’t upload

  • I followed all of the instructions to upload a .csv file. I saved it as a CSV file and made sure it used commas, not semi-colons. I put sds in the first header. I got a successful upload message. But when I go to the forms page I just see random numbers as my form name and there is only one blank field and no entries come through. What am I missing?
