CSV Bookings – Attendee Form Field “Column Name”s don’t appears

Hi,
I’m using the trial version of the Events Manager Pro plugin, through an account on the website https://trywpem.com.
In the Admin, via Events>Form Editor I have created an attendee form with 3 fields: Name, Surname and Email; I used this form in an event.
When I used ‘Export these bookings’ (in the Admin) from Events>Bookings page, for that event I got a CSV file that:
1) In the first line presents: ‘Exported bookings on Tue 09 Jan 2018 11:13’
– Is it possible to avoid inserting this line in the csv?
2) In the second line presents the names of the columns / fields: the names of the fields of the attendee module don’t appear (from the third line on, however, there are the values for these fields)
– Is it possible to display the names of the columns / fields of the attendee module?
