CSS Issues with the name field

Two forms on the same site, but one looks weird. The first one (which was the default one) looks ok, but when I duplicated it, the Name field got this ugly background (screenshot: https://i.imgur.com/sRC90r9.png – the one on the left is the default one)
I checked the output code and the one on the right has a background image applied to it.
I can’t figure out why that is the case – every single setting in the two forms is identical on the backend.
