Creation of duplicate Login pages

Hi the plugin created 5 Login pages with the same slug, I can se them in the permalink selection of users ultra settings and in phpmyadmin but not in the page listing neither in the menu and I cannot delete them from phpmyadmin. WordPress will not redirect to the login page it creates a loop and just hang there. I deleted the only page that was listed, created a new one with the login shortcode, a different slug and name, then assigned it to the permalinks and it is the same. In the console I get “Uncaught TypeError: jQuery(…).datepicker is not a function” thank you
