Creating Assessment Form

    I have a very basic understanding of wordpress and its functionalities – It was suggested to me that I check out the forum to see if I can find a solution to my ‘how to’ question…

    I want to create a form that is used as an assessment – for instance, it has a list of skills and you can check which ones you have. At the end of the form I want the results to either be sent to the survey taker, or they are able to print them out.

    The ONLY data I’m really concerned with for myself is how many people have taken the assessment – their responses need to go to them as that is the point of the assessment, to help them think through what they have.

    I’m about to buy a domain and would install WordPress on the site if there was a plug-in/easy way to build this…

    Oh..and I need this functional by Monday Feb 25…

    I would look into Gravity Forms. It’s a premium paid plugin, but I’m 99% sure you can do what you are looking for.
