Creating a PDF from form submission multi page

  • Can I create a multi page custom template for Form to PDF creation.

    I have a Form which has a number of fields in it.

    But the PDF I need to create has 4 pages in it.

    Data from the form would only be used on pages 1 and 3 of the PDF, the final PDF sent to the customer for be a proposal. Pages 2 and 4 of the PDF cover their generic pricing and terms and conditions.
