creating a form to print envelopes

I’d like to create a form with input fields so people can put their name and address as the sender, and the recipient fields would be prefilled in from a database. I don’t want to capture their information, just make it easy for them to print out the envelope to mail a letter.

I can’t find any kind of plugin that will do this. And sadly I don’t know how to do it on my own.

Can anyone give me any kind of help? I’d appreciate it if you can!
