Creating a form to calculate user input and providing an answer

I have zero html knowledge, but am good with excel etc for a doctor.
I have created a excel spreadsheet that takes user input like gender, age, whether they have diabetes, BP, CAD, LVH, AF, etc. It then calculates the risk for stroke for that individual based on scores provided by the AHA.
The formulae in excel are medium complex, though it mostly involves additions and multiplications only. I also need to CONCATENATE a few lines of text, which contains inline values derived from these calculations.
I would like to put this form within a page which will pop up when the user clicks a button.
I was about to purchase Formidable or Gravity Forms, but am still not sure if they will provide me with the functionality I need to create this form. Can anybody help me get the right equipment? The link to the relevant page: https://www.doctornerves.com/stroke-risk-assessment/
