Created custom contact form but dont know how to collect data

In my footer my theme support has created a custom contact form and just left me in the dark. it is a simple collection of opt in phone number. but i do not know how to collect the data when someone puts their number in the input field, also even by clicking submit the form is submitted, need a rule where only a 10 digit number should be allowed to be submitted please please please help
