Create page that displays items/prizes for selection (not purchase).

I am conducting a survey. At the end of the survey, I want to direct respondents to a webpage where they can select their INCENTIVE — choice of posters (about 4-6 posters). There may be other incentives, but this is all I have right now. I want to display the image/posters and include a ‘check box’ under each for selection. Would also need to ask their name and shipping address.

What PLUGIN would work to create this page?

I would need it to also send me the information after each completion so I can fulfill the selection.
