Create Multiple Password Reset forms

I’m wondering if it’s possible to generate more than one password reset form, each with its unique shortcode that I can easily add to any page. Also, I’d like to know how to modify the button URL to redirect the user to a specific page after resetting their password.

Currently, when a user decides to reset there password it redirect them to a specific page, where can i configure the password reset form so that it redirects the user to a separate page?
