Create form

Hi there,

is it possibile to create a form and adding a radio button with 4 options, depending on the user’s choice an email will be sent to a different address.
Example:
Name: Albert
Email: [email protected]
Hotel Choice: Hotel Kent
Submit email to [email protected]

Name: Roger
Email: [email protected]
Hotel Choice: Hotel Blue
Submit email to [email protected]
