Create custom search form and template

For styling form, if you have only 1 form, you can directly styling with the input id, because each input has it own id, such as, tax-select-0,cmf-select-0,etc. So all you need is using your browser inspect element feature to identify the id, and use it for styling. If you have multiple forms, then you can use the form id as the selector. Because each form id is unique as well.
