Create a form for only entering customer data

    Hey,

    I’d like to offer customers the possibility to fill a form with their basic information (name, email + custom fields) and request an appointment without selecting an appoitment date. It would be a staff memeber who would actuall pick a suitable date afterwards.

    Is that possible?
