country and phone no fields are not displaying on ccavenue payment page

Hi,

I have installed this plugin and I able to redirect it to ccavenue page for payment after checkout but country and phone number is not showing in shipping address and i can not add it as these are disabled fields. All other fields are showing up there except these two so can you please help me to fix this issue?
