Costumize form Message text field show no name, companyname etc it stays empty

    Hi Takayuki,

    First of all; many thanks fot this wonderful, powerful plugin. I use this plugin for standard contact form for many websites in WordPress.
    I have a question and I cant find not really a solution on the FAQ on the website, or I over read it on the FAQ page. But I hope that you can help me with this.

    I’m creating a costumized form / orderform for my companywebsite, I put down in the Message text field (on the right) with tags so that’s clear for me who’s someone his/or the name of the new/exist costumer and her/his companyname and it’s clientnumber and place and Zip postal and (cross)street and phone / cellphone number.
    When I test it out, like a testmail it’s turning blanc; only the e-mailadress is showing in the testmail.

    I think I do something wrong, if I’m doing something wrong, I dont know. I’m using additional headers on the left, only with [example]. And in the Message text field for mail I’m using the next tags: <example>.
