Cookie help

Thanks for the great plugin!
I am not sure if I set the cookies properly.
When first visit the the site workasamodel
the pop-up window opens fine.
When data is submitted the window stays closed until browser cache is closed.
But if no data is submitted and the window is closed how do I get the window to re-open – say when a new page is visited or the browser is refreshed?

Also on the form itself how do I have the check boxes show up as checked by default?

last the styling of the check boxes is off with the two dots that stay to the left while all else is centered how do I get them to center as well?
