Content invisible on slide-out

The contact form was working well, but today I made an edit and now the form doesn’t seem to be rendering correctly.

I updated to the most recent plugin, but this is still the case.
I have embedded the form on a temporary internal system, so it’s not available for me to publish a link to this page.
