I’m not really sure is that you want, but I found this to change the style of contact form: https://www.wpsnippet.com/creating-responsive-two-column-form-using-contact-form-7/

Then, you have to choose “Modèle par défaut” as template for your page (it’s translate because I’m french, but I don’t know the exact translation in english, it’s the template with right sidebar) You add your information in widget sidebar, and you use the shortcode google maps to have the map.
