Contact Us Form

Hi,
Is there any Contact Us Form, where the user can select from a drop down box witch contains a list of the department available “Customer Service, General, Support,…” and once he click “Send” button, each department will receive the email sent to him?

Note: Most the plug ins that I found so far, it only send to one email address.
