What plugin are you using?

im doing directly from the edit page, next to “add media” on the left side there’s a little button that says “add a custom form”

This is not default WordPress functionality, so again, what plugin provides this form?

This is most likely from JetPack (A Plugin with several modules)…which is likely not working well with your theme, another plugin, or a combination of both. That plugin’s support is at:
