Contact form … with some editable fields

not sure where this is .. I thought I had it in m favorites .. but could someone point me where I could find that contact form … there was one specifically for WP and another as a stand alone .. that could be used for a more advanced contact form …. one that you could add additional fields required for registration …

I think the domain … had the word “dagon” in it …
