contact form with float option AND upload option???

Hi guys,

Ive been searching and searching, but i cant seem to find anything , but then again, most of the contact forms look the same.

Does anyone know of a contact form that has the option of setting it as a side floater? AND has the option to have an “upload file” box?

I have seen slick contact forms, but that only has room for 3 fields, and there are a few good ones out there with the upload feature, just not both float and upload options.

I think a floating contact us button on my site would look good, and hopefully make people contact us more, rather than having to click on the menu.

Any recommendations for forms with both?
