Contact Form That Saves Logged In Username?

    Hi there

    After scouring the available plugins and trialling about 5 I have got no further.

    The contact form system absolutely needs to have the following functionality:

    (*) embed the logged in username and name to the form
    (*) the selection of a specific option on a menu should change the recipient
    (*) have a database storage feature and maybe a nice friendly system that allows the selected “staff” to log into and view their received forms.

    From what I have seen, not one contact form (that is free) easily allows this functionality. Is there anything (or anyone) out there that can help me get what I have to achieve?
