Contact form that can check data in database

Hello,

I need contact form that have option with a field to enter number which would be checked in database and if number exist, form would redirect to another page and if doesn’t exist it would say something custom.

So my question is, is there any contact plugin with that option or with add-on that can enable this or it need to be written from ground up? If latter option is way to go, which contact form plugin you recommend to use?
