Contact Form Not Working!

    Hello!

    On every page, on right side I have setup a form named “FREEBIES”. I am facing two issues with it now;
    1) On desktop (I have checked on Chrome & Safari browsers only) when clicked on icon it disappears! How to fix that ?
    2) First section is for mail in form, but for second section nothing is showing up even I write some stuff like “comments”. You can check that too!

    Let me know how to fix it.
