Contact form not displaying right

    First let me say I really like this survey plugin very easy to use. I created my survey and me last question is a custom text box and in it I am trying to use the contact form but it does not display correctly. I have tried to make my own with the survey tool but it never saves and even the default form does not display right. Please help
