Contact form in sidebar

Hi!
Whenever I’m using a contact form on my site, I place it indside the siteorigon widget called ‘Harmonika’ in danish. I dont know the english word for that widget, but it is the widget I use here.
I think the look of the contact form is more professional, when placed inside that particular widget.
On some pages I want the contact form (and widget) placed in the sidebar. But it doesn’t work as expected. Check link below.
Am I doing something wrong?
