Contact form & images not showing on one webpage

Hey,

I’ve created a contact form using WP forms & put it on my home page which is working perfectly. I’m now trying to add a form to another page on my website & cannot get it to show. I’m confused as to how it is working perfectly on one page but not the other??

If it’s in any way relevant the “featured image” section is also not showing on this page. I can add posts & change the colours to this particular page no problem but just cannot images or the contact form (which is working fine on the other pages)

Anyone have any suggestions as to what’s going on?
