contact-form-7-to-database-extension Select a Form not Showing a Form

Hello!

One of my forms is not showing up in the database drop down menu, of the database page. I want to export the users that have submitted.

I have confirmed that the form is indeed inserting into the database successfully.

I have also confirmed that I am not blocking it in the settings page.

Please advise! Thanks!
