Contact Form 7 is not showing up on any pages

I have developed my own WordPress theme, and I am using contact form plugins, but the forms are not showing up for some reason. I have tried a couple of plugins Contact Form 7 and Formidable, no luck! Here is the page I am trying to add a form, BestMoldRemovers.com
