    Takayuki,
    Thank you for the reply, although I have followed those instructions as you referenced a number of times although still not attaching. I have tried several forms and tried mailing to a couple different email addresses that I have on different servers and still no go.

    Would it be possible for you or anyone else that might read this post to provide me with a sample form setup that works and I can try it on my server?
