Contact form

Hi,

My “contact us” menu appears twice on my contact form, in two different formats… Does anyone know how I can remove one ? I don’t need contact us to be repeated on the same page… https://www.casthelite.com/contact-us/

Actually, it takes up loads of space too so there is lots of blank..
