Constantly locking me out

I’m using plugin version 4.2.2 and have added my IP to the global whitelist, and user whitelist.
I am still being constantly locked out. The only way to get in after that is to rename the plugin directory, or access the database and manually remove the lockout record.
Why is this happening? Why are the whitelists not being respected?
