Constant Contact integration

Hi, I’m trying to set up a client’s forms and having a problem with it not adding new subscribers to his CC lists. There is one part of the form setup I’m not clear on, which could be causing the issue. Just below and to the right of the shortcode generated by clicking the “generate tag” button that I’m supposed to add to the form html is another, shorter shortcode with the instruction “And, put this code into the Mail fields below.” But which mail field, and where? I placed it in the “Additional Headers” field but this doesn’t appear to be working correctly. I’m also not sure that I’m placing the main shortcode correctly. Currently I have it like this:
