Connect Data Collection form to Simple Content Construction

I just downloaded and set up a data collection form. I want the data to be displayed in a simple table where it can been seen easily. I installed simple content construction kit, but I cannot see how to connect these.
I don’t want the data to come out on a separate page.
My form is at https://www.mainstreet-realty.com/rentals/rental-form/. The password is mainstreet.
