When I click ‘Click here to create a booking form with this calendar’ in the Calendar editor I get:

A fatal error occurred. Please try to refresh the page. If the error persists, follow the process under “Booking Activities doesn’t work as it should” here: FAQ.
Advanced users, you can stop loading and free the fields to try to solve your problem:

No further details. Refresh or ‘Stop loading and free fiels’ does not help. The form itself, when integrated in a page with the shortcode seems to work though.

When I disable this plugin: OSM – OpenStreetMap, the error goes away. OSM itself is working fine.

WP 5.9.4; PHP 7.4; Avada theme; many plugins all latest update.

Thanks for reporting, I add it to the list of known compatibility issues here: https://booking-activities.fr/en/documentation/faq/

Note that you can also create a booking form in Booking Activities > Booking forms > Add new. Then go to Calendar settings > Filters tab > in the “Calendar” option, select the desired calendar(s) to retrieve events from.

Thanks. I see now that ‘Click here to create a booking form with this calendar’ generates a new shortcode each time.

Yes, it creates a new booking form each time.
If you want to edit an existing booking form go to Booking Activities > Booking forms > your form.
