    Thanks. It looks like there is a double noscript-handling from both the form and the lazy load plugin.

    As it looks like it is an iframe element that gets messed up in the handling, you can try to turn off iframe support on the BJ Lazy Load settings page. At least until I can add and exception for your form plugin.
